Board of Health Performance Assessment - April

Recommendation

It is recommended that Report No. 064-12 re Board of Health Performance Assessment - April be received for information.

In November 2010, the Board of Health endorsed the recommendation of the Board Governance Working Group to annually review the effectiveness of the Board’s performance assessment process (Report No. 135-10). Since then, the Board has assessed its performance in March, June and November of 2011, and signed an Accountability Agreement with the Ministry of Health and Long Term Care, which mandates Board compliance with the Ontario Public Health Organizational Standards.

In March 2012, The Board endorsed revisions to the Self-Assessment Process. Under the new process, Board members will review Board of Health performance two times per year in April and November.

Attached as Appendix A(PDF 39.6KB) is the revised survey document.  Board members are asked to complete the online survey or submit a completed paper copy of the survey by April 27, 2012. 

Completed hard copies can be left in a sealed envelope with the Executive Assistant to the Board of Health, Ms. Sherri Sanders, at the April 19^th Board of Health meeting or mailed directly to Ms. Sanders at 50 King St., London, ON, N6A 5L7.

1.   The scale for the survey ranges from Strongly Disagree on the left to Strongly Agree on the right.

2.   Page 6 of the survey, Performance of Individual Board Members, is for personal use only and is not to be submitted.

A summary of the results will be provided at the June 2012 Board of Health meeting.
