Home Visits

Home visits are available to pregnant women and families with children from birth until they’re ready for school through the Healthy Babies Healthy Children (HBHC) program. The Nurse-Family Partnership ® (NFP) program provides home visits to first-time parents, 24 years old and under. Professionals can refer to the Middlesex-London HBHC and NFP programs by faxing the Referral Form (PDF) or calling the Health Unit at 519-663-5317. NOTE: The Referral Form is only meant for professionals to use as a referral form to the program and must have the client's consent. Please save the PDF to your computer before filling out the form.
