Q: Why does the Health Unit ask for your personal information?
A: The Health Unit's Early Years Team asks for your personal information in order to better understand your questions or concerns and help answer questions about you and your child. We also may need to contact you about your care at a later date.
Q: What personal information does the Health Unit collect?
A: You will be asked to provide your name, address, and contact information. You will also be asked questions about you and your baby's physical and mental health.
Q: How does the Health Unit use the personal information it collects?
A: The Health Unit uses this information to provide Early Years services to you and your child including the Healthy Start Infant Drop-ins, Breastfeeding Home Visits and telephone support. Answers are recorded on an electronic client record. Your information is kept secure and confidential.
