Below please find information from the Ministry of Health and Long-Term Care on the influenza vaccination program for 2013-2014.
The attached notice suggests the use of a special order form for ordering influenza vaccine. For Middlesex-London, you can use the standard order form that is used for all vaccines. You can begin ordering additional influenza vaccine as of October 7, 2013. Also note that the attached document reminds health care providers to use the influenza-specific billing codes when billing for influenza vaccine. Information from the Ontario Medical Association on specific billing codes is provided below.
Attached also please find the Influenza Vaccine Information Sheet that we use at our clinics to obtain informed consent and the schedule of our community influenza vaccination clinics. This information is also available on our web site or on our home page at www.healthunit.com under “Hot Topics”.

Flu Shot Billing Reminder
In continuation from the FY2012/13 season, the code Q590A ‘basic flu shot fee-per-visit premium FHN/FHO’ is payable under the Family Health Network (FHN) and Family Health Organization (FHO) contracts for the provision of influenza vaccination where the provision of the influenza vaccine is the sole reason for the visit.
FHN and FHO physicians may submit the Q590A ‘basic flu shot fee-per-visit premium FHN/FHO’ Fee code only in conjunction with G590 influenza vaccinations Fee code. The Q590A is payable for enrolled and non-enrolled patients.
Other physicians (Fee for Service, FHG, CCM, etc.) should continue to submit the G590 with a G700 for sole visits. If the Q590A is submitted by a physician who is not a member of a FHN or FHO group, the claim will reject to the physician’s error report EPA ‘Network Billing Not Allowed’.
The Q590A fee code is not payable in addition to the G700A ‘sole visit premium’ fee schedule code.
For flu shots administered together with an Assessment, the appropriate assessment should be billed together with a G590.
