Instructions for Registering Cooling Tower/System(s) and Updating Information

Please find below the instructions to follow when registering cooling tower/system(s) or updating information.

Step 1.

Print the number of Registration Forms you will need for the Cooling Tower/System(s) you own/operate. See Step 2 and Step 3 below for help figuring out how many of each form you will need.

• Form 3: Cooling Tower Information (PDF 40KB)

Step 2.

For Initial Registration, complete each form listed below as indicated. For Annual Registration Updates skip to Step 3.

• Form 1 (PDF 56KB): Cooling Tower/System Ownership Information and the Acknowledgment must be completed and signed by the Cooling Tower/System owner. Complete one form for each building address.
• Form 2 (PDF 55KB): Cooling System Information must be completed for each cooling system.
• Form 3 (PDF 40KB): Cooling Tower Information must be completed for each cooling tower within a cooling system.

A cooling system is defined as:

• A single cooling tower, or
• A series of cooling towers that share a water recirculation system or common reservoir.

Step 3.

For Annual Registration Updates - Complete all the forms above if any information has changed. If there have been no changes since your last registration, complete and submit only Form 1. The annual registration update is to be sent to the Middlesex-London Health Unit by January 31^st.

Step 4.

Submit the completed forms by mail, fax, or scan and email to:

Additional Information

Once initial registration is complete, you will be contacted to review your Cooling Tower/System(s) Risk Management Plan. If you do not have a Risk Management Plan in place, a Risk Management Plan Template will be provided to help you create a plan. If changes have been noted to a Cooling Tower/System in an annual registration update you may need to update your Risk Management Plan. For more information see Risk Management Plans.
