Background:
The Association of Local Public Health Agencies (alPHa) provided comments regarding the implementation of the Public Health Funding Formula and called for annual cost of living adjustments to grant funding, upward adjustment of the overall Ministry of Health and Long-Term Care allocation for public health programs and implementation of a comprehensive monitoring strategy to evaluate the new funding model.

The Board of Health previously endorsed this correspondence from the Association of Local Public Health Agencies at the December meeting.

Topic: Anticipated Amendments to the Ontario Public Health Standards (OPHS) and related Protocols

Background:
The Ontario Public Health Standards (OPHS) and Protocols establish the minimum requirements for fundamental public health programs and services to be delivered by Ontario boards of health, which include assessment and surveillance, health promotion and policy development, disease and injury prevention, and health protection.

On January 1, 2009, the OPHS and Protocols replaced the Mandatory Health Programs and Services Guidelines (MHPSG), 1997. Additional changes to the OPHS will take place in 2016 as a result of three initiatives: 1) Integrating the Healthy Smiles Ontario Program; 2) Amendments to the Smoke Free Ontario Act; and 3) Implementation of the Electronic Cigarettes Act.

Additional information regarding the impact of these changes to Middlesex-London Health Unit programs and services are highlighted in the Summary Information Report for January 2016.

Topic: Food Security and the Transformation of Social Assistance in Ontario

Background:
All public health units in Ontario conduct a Nutritious Food Basket (NFB) survey in accordance with the Ontario Public Health Standards to measure the cost of basic healthy eating. Annual results repeatedly show that incomes for our most vulnerable residents are insufficient to afford basic needs.

The Board of Health considered a report at the September 17th meeting and approved that the Board: 1) Send a letter to the Prime Minister of Canada, the Premier of Ontario and the Ontario Minister Responsible for the Poverty Reduction Strategy requesting they prioritize consideration and investigation into a joint federal-provincial basic income guarantee; 2) Send a letter to the Premier of Ontario requesting the province increase social assistance rates to reflect the rising cost of nutritious food & safe housing; 3) Send a letter to all London and Middlesex County federal election candidates requesting they take Food Secure Canada’s Eat Think Vote candidate pledge; and 4) Forward Report No. 50-15 re 2015 Nutritious Food Basket Survey Results and Implications for Government Public Policy to the City of London, Middlesex County & appropriate community agencies.

Topic: Association of Local Public Health Agencies (alPHa) Winter 2016 Meetings

Background:
The Association of Local Public Health Agencies (alPHa) seeks to assist local public health units in providing efficient and effective services that meet the needs of the people of Ontario. It also strives to assist in establishing, through collaboration with other organizations, a unified and powerful voice for public health in Ontario.

alPHa will be holding their winter 2016 meetings on February 24th and 25th in Toronto with sessions regarding risk management, recommendations in the Assessors Report on Algoma Public Health Unit, proposals for changes included in Patients First – A Proposal to Strengthen Patient-Centred Health Care in Ontario, and the Council of Ontario Medical Officers of Health section.

Topic: Public Health Funding and Accountability Agreement Indicators

Background:
Under section 5.2 of the Accountability Agreement between the Middlesex-London Board of Health and the Ministry of Health and Long Term Care (MOHLTC), the Board has agreed to use best efforts to achieve agreed upon performance targets for the indicators specified.

2015 year-end reporting has been completed by Middlesex-London Health Unit staff and Ministry verified results will reported to the Board of Health in October.

Changes to the 2016 indicator suite include the transition of some performance indicators to monitoring indicators, and the addition of four new health protection indicators.

Topic: alPHa Update on Patients First

Background:
The Association of Local Public Health Agencies (alPHa) drafted a summary and linked to relevant materials regarding the recent release of the Patients First discussion paper released on December 17, 2015.

The Board of Health received a report at the January meeting discussing the information that is currently available to Public Health Units regarding the Patients First Discussion Paper and informing the Board that they will be kept fully apprised should further information become available.

Topic: Board of Health Resolution – Smoke-Free Multi-Unit Housing
From: North Bay Parry Sound District Health Unit

Background:
The Smoke-Free Ontario Act prohibits smoking in common areas and ensures that signage is posted in appropriate locations. However, people who live in multi-unit housing are at risk of being negatively affected by second-hand smoke from adjacent units. Few buildings designate their units to be smoke-free and tenants can have very little choice in their housing arrangements.

Public health units and organizations like the Non-Smokers Rights Association and Smoke-Free Housing Ontario advocate for tenant protection in these multi-unit dwelling through voluntary no-smoking policies and future development of governmental policy to facilitate the provision of smoke-free housing.

Topic: Ontario Creating New Enforcement Team to Combat Contraband Tobacco

Background:
Contraband cigarettes are a public health issue that undermines the successful implementation of the Smoke Free Ontario Act and strategies intended to reduce tobacco use in our population. For Ontario’s 2015 Provincial Budget, The Association of Local Public Health Agencies noted a commitment to combatting contraband tobacco. One such measure being implemented is the creation of a Contraband Tobacco Enforcement Team within the Ontario Provincial Police.

Topic: Legislation to Enforce Infection Prevention and Control Practices within Invasive Personal Service Settings under the Health Protection and Promotion Act

Currently, there are no infection prevention and control training requirements for personal service setting operators. The recent changes to the Ontario Public Health Standards did not address the potential to implement legal requirements for infection prevention and control training and operator responsibility in personal service settings (PSS). The Haliburton, Kawartha, Pine Ridge District Board of Health is advocating for the province to consider this in the future.

Topic: Cannabis Regulation and Control: Public Health Approach to Cannabis Legalization

Background:
Canada has one of the highest rates of cannabis use in the world. With this in mind, police associations and public health organizations have expressed support for a new approach, and the federal government has indicated that they will legalize cannabis in their current mandate. Cannabis use is associated with a variety of health harms. The most concerning occur among youth and chronic heavy users. A public health approach to cannabis policy is recommended, including a strong policy framework of strict regulations to minimize health and social harms.

The Board considered report No. 003-16 “Cannabis: A Public Health Approach” at the January 21st meeting and: 1) authorize staff to advocate for an evidence-based public health approach to Cannabis in the context of legalization, including strict regulation for the non-medical use of cannabis, as well as its production, distribution, product promotion and sale; 2) establish baseline data and mechanisms to monitor local use of cannabis in the coming years; and 3) forward this report and appendices to the Association of Local Public Health Agencies, the Ontario Public Health Association, Ontario Boards of Health, the Ontario Minister of Health and Long-Term Care, the federal Minister of Health, and other elected officials as appropriate.
